The full IACUC Form can be downloaded directly from this site.  It is available in MS-Word format. Click on the appropriate link below and select "Save Link As ..." in order to download a file to your computer.  The forms used to request an extension or to change an approved procedure are also listed below in either format.  If you are having problems downloading any of these forms, please contact Janice Kampmann by E-mail at Janice_Kampmann@sdstate.edu or by phone at 688-6649.  She can also E-mail the forms to you or send them by campus mail.
